Joining the Practice

 

If you are interested in registering with St. Luke's Medical Centre, please call reception and ask for a registration pack. You will be given forms to fill in and return to the practice, after which we can book you in for a registration appointment with one of our nurses. Alternatively, you can download and print the forms here - please follow the instructions carefully.

On your first appointment they will discuss the approach of the practice and take your medical history. It is not possible to deal with specific medical conditions in this first appointment. If you have a problem that needs treatment, you will be asked to make another appointment.
