Prescriptions

Repeat prescriptions requests should be made in writing and can be sent to us in a number of ways as detailed below.

If you have received a computer printed prescription for medication you are taking regularly, it  should have been given to you with a repeat order form attached. Please tick the items you need and where you would like to collect the prescription from, and send it to the Stroud surgery.

You can also use our Prescription Request Form, which is available at the surgery, or you can download it here. This can then be posted to the Stroud surgery,  faxed to 01453 756573. We now also accept request forms by  e-mail. Send  to prescriptions@stlukesmedicalcentre.org

Please note that while this online form provides a quick method of sending us a request, it does not increase the speed at which we are able to issue your prescription. Requests received before 11.00 am will be ready 2 working days for your prescription to be ready. This does not include postage time if the prescription is to be collected somewhere other than the Stroud surgery. Please note that requests may take longer if we need to contact you about your request.
